Chronic Disease Self-Management Program

Chronic Disease Self-Management Program is a six-week program that teaches older adults, 60 and older, with chronic illnesses the skills needed in the day-to-day management of treatment and to maintain and increase one’s activities.

Using a group setting to give a shared learning experience, the program teaches techniques that will let individuals do the following:

  • Manage Symptoms
  • Incorporate Exercise
  • Monitor Nutrition
  • Talk with Your Doctor
  • Make Informed Treatment Decisions
  • Deal with Emotions
  • Control Stress

This six-week program will be held from 9 to 11:30 a.m. on Thursdays from February 6 to March 12.

This program will be located in the Buda Municipal Building, Building 100.
